  • Copy of Driver's License of State ID

  • Copy of Social Security Card

  • Copy of Car Insurance   (Must be current)

  • Date of Vehicle Inspection Sticker

  • Copy of CNA Certification (if applicable)

  • Fee for Background Investigations $41 (will come out of your first paycheck)

  • Must be willing to work weekends and holidays!!!
