WHO QUALIFIES

Individuals who are disabled according to Medicaid standards or SSI disability criteria, meet Medicaid financial eligibility, imminent risk criteria and meet the criteria for admission to a nursing facility.

Who should apply?People choose to receive Medicaid Waiver Services for a variety of reasons. Some are unable to care for themselves while others are having difficulty caring for their loved one.

 

HOW TO APPLY

First, we recommend that you make sure you qualify. Recipients eligible for Medicaid Waiver Services must be on the Developmental Disability (DD) Service Registry to be considered for services. Names are selected by date/time order to the earliest request. To ensure your name is on the list or if you are eligible for services call the BCSS helpline at 1-866-783-5553.

Community Choice Waiver provides services in the home and in the community to elders or adults wo are 21 years old or older with disabilities who qualify. Personal Care Assistance with basic self-care activities or tasks such as dressing, bathing, preparing meals and so forth.

 

PERSONAL CARE SERVICES

 

EPSDT/PCS Services (under 21 years of age with Medicaid) Services include basic personal care tasks which include bathing, dressing, grooming, toileting, eating, meal preparation and incidental household chores that the recipient requires.

 

Long Term - Personal Care Services (22 and older) Provides services for eating, dressing, grooming, bathing, transferring, ambulation, light housekeeping, laundry, meal preparation, doctor visits, scheduling appointments and setting up transportation.
